We have 3 types of health forms available for download online:

Physical exam

Dental exam

Eye exam

Don't know which one you need?

Kindergarten:  you will need all three.  The immunization section of the physical form MUST be filled out.  

2nd grade:  You will need the  Dental exam form.

6th grade:   You will need the  Dental exam form.  Also, if you did not provide a Hepititis B immunization record when the student was in 5th grade, you will also need the physical exam form with the immunization section completed.

5th - 8th grade sports:  You will need to provide the  physical exam form each year that sports are played.

DIDN'T GET A CHANCE TO REGISTER YOUR KIDS FOR 08-9 YET?   There are two types of registration that need to be completed;  OAK GROVE SCHOOL registration and PTO registration.

School registration will open again in August

PTO registration, however, can still be done via mail.  The PTO provides everything except for OGS registration fees, band fees, milk fees, and PE uniforms.  ALL OTHER ITEMS (i.e. lunches, directories, Pre-packs) are purchased through the PTO.

CLICK HERE to download the necessary PTO forms.  You can fill them out, calculate your amount due, and send a check into the PTO drop box. All instructions are in the PTO forms.

When calculaing PTO fees, please include the $20 late fee that the PTO charges for registration after 6/11.

All current K-7 students should have received (or will shortly receive) a registration letter for the 08-09 school year.   

 As the letter indicates, parents are STRONGLY being encouraged to register online.  If you choose to register in person, however, you can save time by downloading the necessary PTO forms ahead of time.  You can get them here:

 Online registration will open up on Monday, May 26th at 8:00 a.m. and will remain open until June 9th at 11:59 p.m.
